Who pays for the ChemoFx test?
ChemoFx has been reimbursed by most major insurance companies and currently receives payment from Medicare for gynecologic cancers.

What happens if you're denied coverage?
We'll do everything in our power to collect payment from your insurance company, so you can focus on your treatment—not your finances. However, if you are ultimately denied coverage, we encourage you to apply for discounts under our Compassionate Care Program. Call us toll free at 1-888-522-7087 or email us for more information.

Your responsibilities
Patients are responsible for applicable co-payments, deductibles and co-insurance as required by your policy.

If your insurance company denies your claim and requires a signed release form in order to process your appeal, we will send you the form in order to ensure that we can handle all of the details on your behalf. Please sign the form and return it to Precision Therapeutics as soon as possible.
